How to make your life or income protection insurance claim

    Check your insurance policy

    Read through your policy documents. The following documents will tell you what you’re covered for, what you’re entitled to claim, and any terms and conditions you should know about:

    • Policy Schedule
    • Product Disclosure Statement (PDS)

    Reach out to us and we'll provide you with the right form for your claim.

    Send the form to us

    Sometimes we need to know a little bit more information to complete the claims process. After reviewing your form, we may ask you to provide medical evidence, a death certificate, or other important documents.
